Dysphagia is common and costly. Complications include malnutrition, dehydration, social isolation, depression, pulmonary abscess, pneumonia, and death. Common causes of swallowing difficulty include gastro-esophageal reflux, cricopharyngeus muscle dysfunction and Zenker's diverticulum, esophageal webs and rings, head and neck cancer, eosinophilic esophagitis, esophageal motility disorders, advancing age, and neurodegenerative disease. It is imperative for the otolaryngologist to have a thorough understanding of swallowing disorders from lips to stomach. This two-hour session will provide a comprehensive review for both generalists and subspecialists. The diagnostic curriculum will include validated patient reported outcome measures (PROM), flexible endoscopic evaluation of swallowing (FEES), trans-nasal and per-oral esophagoscopy & gastroduodenoscopy, ambulatory pH and impedance testing, swallowing fluoroscopy, and high-resolution impedance manometry (HRIM). State-of-the-art management strategies to be discussed will include advanced techniques of dilation, endoscopic and open myotomy and diverticulectomy, use of botulinum toxin, hiatal hernia repair, and radio frequency delivery for endoscopic reflux management and treatment of intestinal metaplasia and dysplasia.
