Since the advent of balloon sinus dilation, there has been a proliferation of office-based techniques utilized in the treatment of diseases that affect the nose and paranasal sinuses. In order for these technological advances to be successful, they must be appropriately applied based on disease- and patient-specific factors. This first part of this session will focus on the selection of patients and the pre-authorization process that is required to ensure reimbursement. This will include a discussion of how to counsel patients in preparation for the planned procedure. The session will then address the various anesthetic methods used in the awake patient in order to ensure patient comfort, including the administration of systemic and topical agents. Technical advances in nasal obstruction, including subcutaneous stenting and radiofrequency ablation of the nasal valve, will then be reviewed. This will consist of technical descriptions of these procedures and a discussion of the expected outcomes and potential complications. Non-resorbing steroid-eluting implants have recently gained favor in the long-term management of chronic rhinosinusitis with nasal polyposis. The session will conclude with a review of the data and a case-based discussion of these adjuvant therapies and address the maintenance issues that may arise while they are in place.
