This panel serves to provide a focused review of the current guidelines based on NCCN recommendations for management of HPV+ oropharyngeal cancer. Using a case-based discussion format, the panelists highlight major controversies relating to the management of these patients based on the most up to date research. The topic is timely and relevant, as there are new studies enrolling patients and entering into the published literature with great frequency, some of which are supportive of primarily surgical approaches for this patient population and others that suggest surgery is only useful in limited settings. Additionally, given the anticipated continued increase in the prevalence of HPV mediated oropharynx cancers, Otolaryngologists will continue seeing more and more of these patients in their practices. While the panel highlights treatment strategies that are implemented at major cancer centers, it also provides comprehensive otolaryngologists with a better sense of appropriate work-up for these patients, when referral to tertiary care centers is necessary, and why close discussion with other multi-disciplinary providers is crucial. To that end, one of the involved panelists is a Radiation Oncologist who provides good insight into the thought processes of our Radiation Oncology colleagues when developing treatment plans for these patients. Further, the inclusion of a non-surgeon on this panel leads to a more insightful discussion on the merits of non-surgical treatment strategies versus primarily surgical treatment paradigms.
