As the airway experts in children, Pediatric Otolaryngologists treat multiple levels of airway obstruction including stenosis within the nasal cavities, midface hypoplasia, and micrognathia. Numerous techniques exist for treatment of airway obstruction, but goals are to relieve the airway obstruction, make feeding more efficient, and improve quality of life. Distraction osteogenesis is a technique that generates new bone formation, allowing for rapid growth and correction of the underlying craniofacial skeleton. This technique has been adapted for use in a variety of craniofacial abnormalities to improve upper airway obstruction. This panel will include in-depth discussion the variety of distraction techniques to treat obstruction at different airway levels. The panel will discuss airway management that might be necessary with the following procedures: mandibular distraction, infant surgically assisted rapid palatal expansion (iSARPE), maxillary distraction, and craniosynastosis, including emergency airway management, indications for surgery, work-up that should be completed, and controversies in management.
