Cochlear implant surgery can pose unique challenges when variant anatomy or prior surgery is encountered. Specific examples include posteriorly displaced round window, prior vestibular schwannoma surgery, aberrant facial nerve, and inner ear malformations. We discuss several cases and provide tips for intraoperative management.
