This course will focus on patient selection and treatment with hypoglossal nerve stimulation therapy. Drug induced sleep endoscopy videos will be demonstrated showing both candidates that are appropriate and candidates that are inappropriate for hypoglossal nerve stimulation therapy. Other aspects of patient history that help identify ideal patients will be discussed. Clearly, patients who have been unable to use CPAP are the best candidates for hypoglossal nerve stimulation therapy as opposed to patients who are using CPAP and are seeking alternatives. This course will discuss surgical anatomy and surgical technique related to all aspects of the procedure. Marginal mandibular nerve anatomy is crucial to avoid inadvertent injury. Surgical tips for easy access to the hypoglossal nerve will be discussed. Techniques using both macroscopic and microscopic dissection of the hypoglossal nerve branches will be demonstrated. Anatomy of the chest wall with specific attention to the intercostal muscles will be reviewed since this is a new territory for otolaryngologists. Simplified placement of the chest pulmonary sensor will be described. Potential complications encountered by the presenters will be described and discussed, along with treatment of the complications. Discussion on troubleshooting when problems arise, correction of identified problems, and corrective procedures will also be included. Follow-up care and inspire adjustments will be discussed and explained.
